Our Services

Harvest Healthcare Solutions serves diverse clients, including homeless individuals, at-risk youth, seniors, immigrants, and refugees, by breaking down traditional barriers to healthcare and housing. Enhanced Care Management (ECM) and Community Systems coordinate in-person care where clients live, bridging the gap between healthcare, housing, and essential services.

As of March 2024, Harvest Healthcare Solutions is now contracted with Medi-Cal Managed Care Plans to provide Enhanced Care Management (ECM) and Community Supports under the California CalAIM Program. HHS utilizes Lead Care Managers to assist Medi-Cal recipients to better access services to promote their health care needs.

Alta Regional CFS Services

We provide CFS services through Alta Regional for adults (18+) with disabilities who live at home with their families.

We currently service the following areas:

Sacramento, Yolo, Yuba, Placer, and Sutter counties.

Justice Involved Population

We partner with correctional facilities across California to support justice-involved individuals as they prepare for reentry and reintegrate into their communities.

Housing Navigation Services (CS

Services that address members’ health-related social needs, help them live healthier lives, and avoid higher, costlier levels of care.

Enhanced Care Management (ECM)

A new statewide Medi-Cal benefit available to eligible members with complex needs.

Senior Placement Services (SP)

This includes living arrangements for assisted living facilities, in-home care support, memory care, board and care, as well as independent living in all 50 states.

Community Health Worker (CHW)

Preventive health services to prolong life; and promote physical and mental health and well-being.
