Alta Regional CFS Services

We proudly provide Coordinated Family Support (CFS) services through Alta Regional for adults (18 and older) with disabilities who live at home with their families.

Our team is here to support individuals and their families by connecting them with services and resources that promote greater independence and improve everyday life.

Support After Incarceration

Starting over can be challenging, but you don't have to do it alone.

We work with correctional facilities throughout California to help individuals prepare for life after incarceration. Our team connects members with housing, healthcare, community resources, and ongoing support to help make the transition back into the community as smooth as possible.

Our goal is to help you build a strong foundation for a healthier, more stable future.

Housing Navigation Services (CS

Finding a place to live can be overwhelming, but you don't have to do it alone.

Our Housing Navigation team helps you find safe, stable housing and supports you throughout the process. We'll help you understand your housing options, complete applications, connect with landlords, and find community resources that can make it easier to stay housed.

Having a safe place to call home can improve your health, reduce stress, and help you focus on what matters most.

Enhanced Care Management (ECM)

If you have Medi-Cal and qualify for ECM, you'll be paired with a dedicated Care Manager who will help connect you to healthcare, housing, transportation, and other community resources.

We're here to make it easier for you to get the care and support you need.

Senior Placement Services (SP)

Finding the right care for yourself or a loved one can feel overwhelming. We're here to help you explore your options and find the best fit.

We can help connect you with assisted living, memory care, board and care homes, independent living communities, and in-home care services in all 50 states.

Community Health Worker (CHW)

Our Community Health Workers help connect you with resources, support, and services that improve your health and well-being.

We’re here to provide guidance, answer questions, and help you take steps toward a healthier life.
