Recuperative Care (Medical Respite)
What Is Recuperative Care?
Recuperative Care is a short-term residential program that provides a safe place for individuals to recover after a hospital stay when they are not sick enough to remain hospitalized but still need support to heal. It is designed for people who may be experiencing homelessness, housing instability, or who do not have a safe place to recover.
Who May Qualify?
Individuals may be eligible if they:

Have recently been discharged from a hospital or are at risk of hospitalization

Need a safe place to recover from a medical condition

Live alone without adequate support

Are experiencing homelessness or housing instability

Have medical, behavioral health, or substance use conditions that make recovery difficult without additional support
Services Provided
Program Requirements
Participants may receive:

Safe temporary housing

Nutritious meals
-
Medical and behavioral health monitoring

Assistance with daily living activities (such as bathing, dressing, and mobility)

Transportation coordination for medical appointments

Case management and care coordination
-
Help accessing benefits and community resources

Housing navigation and support for long-term housing goals
To enroll, individuals generally must:

Have a medical need for recuperative care

Be medically stable and no longer require inpatient hospital care

Need short-term support to recover safely and avoid hospital readmission

Be referred by a healthcare provider, hospital, health plan, or other approved referral source

Meet eligibility requirements established by their health plan or funding program
Length of Stay
Recuperative Care is intended as a temporary service. Stays are based on individual medical needs and generally do not exceed 90 days.
