What Is Recuperative Care?
Need a safe place to recover after leaving the hospital?
Our Recuperative Care program gives you a safe place to stay while you heal. It is for people who are well enough to leave the hospital but still need extra support before returning home.
This program is especially helpful if you are experiencing homelessness, housing instability, or do not have a safe place to recover.
Who May Qualify?
You may qualify if you:

Recently left the hospital or are at risk of going back

Need a safe place to recover from an illness, injury, or surgery

Live alone and do not have someone to help you

Are experiencing homelessness or do not have stable housing

Have physical health, mental health, or substance use needs that make recovery harder
What We Provide
How to Get Started
While you stay with us, you may receive:

A safe place to stay

Healthy meals

Help with, dressing, and other daily activities if needed

Support from our care team

Transportation to medical appointments

Case management
-
Help applying for benefits and community resources

Help finding permanent housing
Our goal is to help you recover safely and prepare for a successful return to the community.
To qualify, you will generally need to:

Have a medical need for Recuperative Care

Be ready to leave the hospital but still need short term support
-
Be referred by a hospital, doctor, health plan, or another approved provider

Meet your health plan's eligibility requirements

How Long Can I Stay?
Recuperative Care is a short term program. Most stays are based on your medical needs and can last up to 6 months
