Housing & Community Supports Services

We help members secure safe, long-term housing. Our team assists with housing deposits, applications, and landlord connections.

 

We focus on smooth transitions from shelters or hospitals into stable homes while ensuring continued access to essential services that promote independence, security, and well-being. We can also assist with employment services if that is a barrier to obtaining housing.

Transitional Housing

Harvest is proud to announce a powerful step forward in our mission of transformative care: we now have our very own STPH homes. This milestone allows us to provide a more personalized, hands-on approach to case management than ever before. By creating a stable, supportive living environment under our direct guidance, we’re able to walk alongside each individual more closely building trust, responding in real time to needs, and tailoring services that truly make a difference.

 

These homes are more than just housing they are spaces of growth, dignity, and opportunity, where individuals can receive the focused support they need to thrive and move confidently toward long-term stability.

Short-Term Post-Hospital Housing (STPHH)

Short-Term Post-Hospital Housing (STPHH) provides safe, temporary housing along with ongoing support to help individuals recover and stabilize. Services focus on helping members regain daily living skills, access needed medical, mental health, or substance use treatment, and receive case management. Members are also supported in starting the process of finding longer-term housing, including Housing Transition and Navigation Services.

 

STPHH can also support individuals who have been recently released from jail or prison and need a safe place to recover and reconnect to care.

 

Eligibility

 

To be eligible, an individual must:

  • Have ongoing physical or behavioral health needs, as determined by a qualified health professional, that would otherwise require continued institutional care without STPHH services, or

  • Be recently released from jail or prison and in need of supportive housing and care.

Housing Setting

 

STPHH may be provided in a private or shared interim housing setting where residents receive the supportive services listed above.
