Clients We Serve
Harvest Healthcare Solutions provides personalized, community-based care for people facing complex challenges, including those recently incarcerated, experiencing homelessness, living with serious mental illness (SMI) or substance use disorder (SUD), involved in child welfare services, pregnant or postpartum, single parents needing support, and individuals with intellectual or developmental disabilities.
We also assist survivors of domestic violence, sexual assault, human trafficking, and families of homicide victims with trauma-informed services. Our Enhanced Care Management (ECM) and Community Support programs deliver in-person care where clients live, connecting them to healthcare, housing, and essential resources. We break down barriers to stability and improve quality of life for high-need individuals.

Hello, I’m Lea, the proud founder of Harvest Healthcare Solutions LLC. My journey in the realm of healthcare began with a deeply personal experience caring for my Great Grandmother Dessie. In her final years, my mother, sister, and I devoted ourselves to her well-being, facing the challenges of caregiving without the knowledge of available resources. Inspired by Dessie’s legacy, I founded Harvest Healthcare Solutions LLC to bridge the information gap and provide support for individuals navigating the complexities of healthcare and housing.
At Harvest Healthcare Solutions LLC, we believe in fostering a compassionate and informed approach to healthcare. Our commitment extends beyond providing services; it’s about building a community that understands the unique challenges faced by individuals dealing with healthcare and housing needs. Anthony is passionate about art, self-care, sports, plants, and fashion. He grew up in the San Francisco Bay Area and deeply values the diversity that shapes our community. He earned his bachelor’s degree in Psychology and has found meaningful purpose in applying both his education and personal experience to support underserved populations.
He currently lives in Contra Costa County, where he contributes to several homeless programs. He's committed to community building and connecting individuals with the resources they need to feel supported and empowered. Before transitioning into the Human Services field, he spent 6.5 years with Kaiser Permanente in the Home Health and Hospice Department, where he gained valuable insight into patient care and support services.

Clinique brings a strong background in behavioral health, special education, and community outreach to her role at Harvest Healthcare Solutions. With years of experience supporting individuals and families in navigating complex care systems, she is dedicated to connecting members with the resources and programs they need to thrive.
Clinique compassionate approach, combined with her knowledge of behavioral health services, ensures that every client receives personalized guidance and meaningful support on their path to better health.

Diana
Patient Care Navigator
Diana is proud to be a bilingual, first generation college graduate. She earned her Bachelor of Science in Family Studies and Human Development from Sacramento State University.
With over two years of experience connecting individuals and families to essential resources, she is deeply passionate about advocacy and approach her work with compassion and a genuine desire to make a positive impact in others' lives. In her free time, she enjoys cultivating experiences that inspire her and fuel her dedication for helping others.

DiOria
Patient Care Navigator
DiOria is a new case manager here at Harvest Healthcare Agency. She graduated from Humboldt State University with a Bachelor of Social Work in 2021.
She's been volunteering and working in the field of Social Work for 6 years. She enjoys assisting people in need and providing them with resources to live impactful lives..
Fay
ECM/CS Supervisor
Fay has over 20 years of experience working with the public, specializing in providing resources and transportation for low-income and homeless populations. She is passionate about helping her clients and finds great fulfillment in her work.
In her free time, Fay enjoys spending quality time with her family.
Jesus
Patient Care Navigator
Jesus graduated from California State University East Bay with a Bachelor of Arts in Psychology in 2022. He has work experience in Applied Behavior Analysis and in Mental Health, with a focus on helping individuals with Major Depressive Disorder, Generalized Anxiety Disorder and Schizophrenia.
He is passionate about helping individuals in getting the assistance they need, making sure that they are well cared for, and making sure they know that there is always someone there to help them.
Judie
Patient Care Navigator
Judie brings a diverse professional background to our company with an MA in International Policy Studies and practical work experience in industries such as: hospitality, medical, nonprofit, and even wildland firefighting. She is part of our Monterey County team where she has integrated into the Monterey area over the past decade, working with community leaders and the people they serve.

As a first-generation American, Judie approaches her role as Case Manager with a social work lens that captures the full depth of culture and conflict as they shape and form policy decisions. Her greatest assets include international business development, capacity building at the micro level - especially vulnerable populations, and a deep connection to the local communities along the Peninsula and Salinas areas.

Liza
Executive Recruiter
Liza has spent the past 20 years as an HR professional, specializing in executive placement within the hospitality industry and recruitment and staffing for healthcare in support of government contracts in healthcare.
She later transitioned into the financial services industry as an insurance broker helping families plan, build and protect their legacies.

Liza’s natural passion for helping people and being a resource for others is a core part of who she is. She is empowered by the spirit of enterprise and feels deeply committed to supporting small businesses and women entrepreneurs.

Marie is a dedicated professional born and raised in the vibrant Philippines. With a Bachelor's degree in International Studies and Tourism,
Marie has cultivated a diverse skill set that compliments her passion for communication and community service.

Lead Care Manager
Ruth plays a vital role in helping patients navigate the complexities of the healthcare system. She acts as a bridge between patients and healthcare providers, ensuring smooth communication with overall client support. Helping clients overcome barriers to care such as transportation, insurance issues and/or financial constraints.

With 5 years of social work background as an administrative assistant for non-profit organizations she has gained familiarity with community resources, healthcare system, referral processes, and patient rights.

Her empathy and compassion guide's her to understand and provide resources with emotional support during stressful times to clients. Utilizes Culture Humility to understand and respect diverse socioeconomic, religious backgrounds to provides care that is appropriate to individual needs. She is resilient and patient when dealing with high-stress situations to remain persistent in advocating for patient needs.
