At Harvest Healthcare Solutions, our mission is to cultivate a healthier future by delivering innovative, patient-centered healthcare and housing services. We are committed to enhancing the quality of care and living conditions through compassionate service. 

 

Our goal is to empower healthcare providers and patients alike, ensuring access to effective healthcare resources and secure supportive housing resources that promotes well-being and drives positive outcomes for communities worldwide.

Clients We Serve

Harvest Healthcare Solutions provides personalized, community-based care for people facing complex challenges, including those recently incarcerated, experiencing homelessness, living with serious mental illness (SMI) or substance use disorder (SUD), involved in child welfare services, pregnant or postpartum, single parents needing support, and individuals with intellectual or developmental disabilities. 

 

We also assist survivors of domestic violence, sexual assault, human trafficking, and families of homicide victims with trauma-informed services. Our Enhanced Care Management (ECM) and Community Support programs deliver in-person care where clients live, connecting them to healthcare, housing, and essential resources. We break down barriers to stability and improve quality of life for high-need individuals.

Lea

Hello, I’m Lea, the proud founder of Harvest Healthcare Solutions LLC. My journey in the realm of healthcare began with a deeply personal experience caring for my Great Grandmother Dessie. In her final years, my mother, sister, and I devoted ourselves to her well-being, facing the challenges of caregiving without the knowledge of available resources. Inspired by Dessie’s legacy, I founded Harvest Healthcare Solutions LLC to bridge the information gap and provide support for individuals navigating the complexities of healthcare and housing.

 

At Harvest Healthcare Solutions LLC, we believe in fostering a compassionate and informed approach to healthcare. Our commitment extends beyond providing services; it’s about building a community that understands the unique challenges faced by individuals dealing with healthcare and housing needs. We strive to be a beacon of support, offering guidance, resources, and personalized care solutions to ensure that every individual's healthcare journey is marked by understanding, dignity, and comfort.

Brenda is a seasoned Funding Specialist with an MA in Rural Sociology and Community Development. Her experience includes more than  12 years in program and portfolio management, funding development, and strategic planning, including pre-and post-grant management.

Patient Care Navigator

Brisette is currently completing her Bachelor of Arts in Sociology with a minor in Social Work, which guides her strengths-based and client-centered approach to care. She brings over ten years of experience in customer service and retail, along with volunteer work for nonprofit organizations, which has strengthened her ability to communicate effectively, build trust, and support individuals with diverse needs. She is  passionate about health equity and advocacy, and she is committed to empowering underserved communities by helping clients navigate complex systems with dignity, compassion, and respect."

DiOria

Patient Care Navigator

DiOria is a new case manager here at Harvest Healthcare Agency. She graduated from Humboldt State University with a Bachelor of Social Work in 2021.

 

She's been volunteering and working in the field of Social Work for 6 years. She enjoys assisting people in need and providing them with resources to live impactful lives..

Fay has over 20 years of experience working with the public, specializing in providing resources and transportation for low-income and homeless populations. She is passionate about helping her clients and finds great fulfillment in her work.

Patient Care Navigator

Felix Foster brings over 30 years of experience working with the public and in the transportation industry, demonstrating a strong commitment to service, reliability, and community engagement.

 

In addition, he spent more than a decade as a care provider in a group home setting, supporting and mentoring men with compassion and dedication. Outside of his professional work, Felix enjoys music and sports, reflecting his energetic and well-rounded personality.

Operations Manager

Liza has spent the past 20 years as an HR professional, specializing in executive placement within the hospitality industry and recruitment and staffing for healthcare in support of government contracts in healthcare.

 

She later transitioned into the financial services industry as an insurance broker helping families plan, build and protect their legacies.

Liza’s natural passion for helping people and being a resource for others is a core part of who she is. She is empowered by the spirit of enterprise and feels deeply committed to supporting small businesses and women entrepreneurs.

Marie is a dedicated professional born and raised in the vibrant Philippines. With a Bachelor's degree in International Studies and Tourism,

 

Marie has cultivated a diverse skill set that compliments her passion for communication and community service.

Over the past decade, she has honed in on her expertise in social work, focusing primarily on individuals with intellectual disabilities and the elderly. Her commitment to this field has driven her to advocate for those who often go unheard, ensuring they receive the support and care they deserve.

Naomi

Licensed Clinician

Naomi is a Licensed Clinical Social Worker with over 30 years of experience in the mental health field.  She retired in 2015 from her role as Coordinator of Substance Abuse Program at Alameda Medical Center.

Her background includes psychotherapist, medical social worker, Children’s Hospital Trauma Program and substance abuse. She also served as a consultant for California Prevention and Education Project. 

​

She is deeply committed to supporting individuals through mental health services and promoting emotional well being across diverse populations.

Ruth

Lead Care Manager

Ruth plays a vital role in helping patients navigate the complexities of the healthcare system. She acts as a bridge between patients and healthcare providers, ensuring smooth communication with overall client support. Helping clients overcome barriers to care such as transportation, insurance issues and/or financial constraints.

With 5 years of social work background as an administrative assistant for non-profit organizations she has gained familiarity with community resources, healthcare system, referral processes, and patient rights.

​

Her empathy and compassion guide's her to understand and provide resources with emotional support during stressful times to clients.  Utilizes Culture Humility to understand and respect diverse socioeconomic, religious backgrounds to provides care that is appropriate to individual needs. She is resilient and patient when dealing with high-stress situations to remain persistent in advocating for patient needs.
