Our Enhanced Care Management (ECM) team helps you get the care and services you need to stay healthy and move forward.

• Find a doctor, dentist, or mental health provider

• Schedule appointments

• Arrange transportation

• Connect with local resources and community programs

• Understand your medications and how to take them safely

Your case manager will work with you one on one to make sure you get the support you need. We're here to help you every step of the way.
