Mga Serbisyong Sumusuporta sa Pabahay at Komunidad
Tinutulungan namin ang mga miyembro na makakuha ng ligtas at pangmatagalang pabahay. Ang aming koponan ay tumutulong sa mga deposito sa pabahay, mga aplikasyon, at mga koneksyon sa panginoong maylupa upang suportahan ang mga indibidwal lalo na ang mga nakatatanda, mga taong may kapansanan, ang malalang sakit, at mga biktima ng krimen.
Nakatuon kami sa maayos na paglipat mula sa mga shelter o ospital patungo sa mga matatag na tahanan habang tinitiyak ang patuloy na pag-access sa mahahalagang serbisyo na nagtataguyod ng kalayaan, seguridad, at kagalingan. Maaari din kaming tumulong sa mga serbisyo sa pagtatrabaho kung ito ay isang hadlang sa pagkuha ng pabahay.

Eligibility
You may qualify if you:

Recently left the hospital and still need extra support

Have physical or mental health needs that make it hard to recover on your own

Recently left jail or prison and need a safe place while reconnecting with healthcare and other services
While you're staying with us, we can help you:

Recover in a safe and supportive home

Connect with doctors, mental health providers, or substance use treatment

Meet regularly with a case manager

Learn or rebuild everyday living skills

Find permanent housing
You may stay in a private or shared home, depending on availability and your needs.
