Harvest Healthcare Solutions ofrece atención personalizada y comunitaria a personas que enfrentan desafíos complejos, incluyendo a quienes han estado recientemente encarcelados, quienes se encuentran sin hogar, quienes viven con enfermedades mentales graves (EMG) o trastornos por uso de sustancias (TUS), quienes están involucrados en servicios de bienestar infantil, quienes están embarazadas o en el período posparto, padres solteros que necesitan apoyo y personas con discapacidades intelectuales o del desarrollo.
También brindamos asistencia a sobrevivientes de violencia doméstica, agresión sexual, trata de personas y a familiares de víctimas de homicidio mediante servicios especializados en trauma. Nuestros programas de Gestión de Cuidados Intensivos (ECM) y Apoyo Comunitario ofrecen atención presencial en el domicilio de nuestros clientes, conectándolos con servicios de salud, vivienda y recursos esenciales. Eliminamos las barreras que impiden la estabilidad y mejoramos la calidad de vida de las personas más necesitadas.

Hola, soy Lea, la orgullosa fundadora de Harvest Healthcare Solutions LLC. Mi trayectoria en el ámbito de la salud comenzó con una experiencia muy personal: cuidar de mi bisabuela Dessie. En sus últimos años, mi madre, mi hermana y yo nos dedicamos a su bienestar, afrontando los retos del cuidado sin conocer los recursos disponibles. Inspirada por el legado de Dessie, fundé Harvest Healthcare Solutions LLC para facilitar el acceso a la información y brindar apoyo a las personas que se enfrentan a las complejidades de la atención médica y la vivienda.

Nuestro compromiso va más allá de brindar servicios; se trata de construir una comunidad que comprenda los desafíos únicos que enfrentan las personas con necesidades de atención médica y vivienda. Brenda is a seasoned Funding Specialist with an MA in Rural Sociology and Community Development. Her experience includes more than 12 years in program and portfolio management, funding development, and strategic planning, including pre-and post-grant management.

Brisette is currently completing her Bachelor of Arts in Sociology with a minor in Social Work, which guides her strengths-based and client-centered approach to care. She brings over ten years of experience in customer service and retail, along with volunteer work for nonprofit organizations, which has strengthened her ability to communicate effectively, build trust, and support individuals with diverse needs. She is passionate about health equity and advocacy, and she is committed to empowering underserved communities by helping clients navigate complex systems with dignity, compassion, and respect."

DiOria is a new case manager here at Harvest Healthcare Agency. She graduated from Humboldt State University with a Bachelor of Social Work in 2021.
She's been volunteering and working in the field of Social Work for 6 years. She enjoys assisting people in need and providing them with resources to live impactful lives..

Fay has over 20 years of experience working with the public, specializing in providing resources and transportation for low-income and homeless populations. She is passionate about helping her clients and finds great fulfillment in her work.
In her free time, Fay enjoys spending quality time with her family.
Felix
Patient Care Navigator
Felix Foster brings over 30 years of experience working with the public and in the transportation industry, demonstrating a strong commitment to service, reliability, and community engagement.
In addition, he spent more than a decade as a care provider in a group home setting, supporting and mentoring men with compassion and dedication. Outside of his professional work, Felix enjoys music and sports, reflecting his energetic and well-rounded personality.

Haya is a highly motivated and experienced cold caller with 6 years of experience in real estate and sales. Dedicated and results driven and has a track record of consistently meeting and exceeding sales targets.
Liza
Operations Manager
Liza has spent the past 20 years as an HR professional, specializing in executive placement within the hospitality industry and recruitment and staffing for healthcare in support of government contracts in healthcare.
She later transitioned into the financial services industry as an insurance broker helping families plan, build and protect their legacies.
Liza’s natural passion for helping people and being a resource for others is a core part of who she is. She is empowered by the spirit of enterprise and feels deeply committed to supporting small businesses and women entrepreneurs.

Marie is a dedicated professional born and raised in the vibrant Philippines. With a Bachelor's degree in International Studies and Tourism,
Marie has cultivated a diverse skill set that compliments her passion for communication and community service.
Over the past decade, she has honed in on her expertise in social work, focusing primarily on individuals with intellectual disabilities and the elderly. Her commitment to this field has driven her to advocate for those who often go unheard, ensuring they receive the support and care they deserve.

With experience spanning radio, media, and digital spaces, Monique understands both audience and impact. Her work in marketing and web design is rooted in strong community outreach and creating environments where people feel seen, supported, and engaged.
Naomi
Licensed Clinician
Naomi is a Licensed Clinical Social Worker with over 30 years of experience in the mental health field. She retired in 2015 from her role as Coordinator of Substance Abuse Program at Alameda Medical Center.
Her background includes psychotherapist, medical social worker, Children’s Hospital Trauma Program and substance abuse. She also served as a consultant for California Prevention and Education Project.
She is deeply committed to supporting individuals through mental health services and promoting emotional well being across diverse populations.
Ruth
Lead Care Manager
Ruth plays a vital role in helping patients navigate the complexities of the healthcare system. She acts as a bridge between patients and healthcare providers, ensuring smooth communication with overall client support. Helping clients overcome barriers to care such as transportation, insurance issues and/or financial constraints.
With 5 years of social work background as an administrative assistant for non-profit organizations she has gained familiarity with community resources, healthcare system, referral processes, and patient rights.
Her empathy and compassion guide's her to understand and provide resources with emotional support during stressful times to clients. Utilizes Culture Humility to understand and respect diverse socioeconomic, religious backgrounds to provides care that is appropriate to individual needs. She is resilient and patient when dealing with high-stress situations to remain persistent in advocating for patient needs.
